ban mit Placebo verglich, senkten beide
Dosierungen das Risiko für erneute VTE
und dadurch bedingte Todesfälle um#$%.
Mit dieser neuen Therapieoption könnten
sich viele Herausforderungen einer War-
farin-Therapie wie engmaschigesMonito-
ring und Interaktionen mit Lebens- und
Arzneimitteln umgehen lassen [#].
Spannende Erkenntnisse
zu neuen Substanzen
aus Phase-II-Studien
Als besonders bedrohlich hat sich bei der
AML die Mutation FLT'-ITD herausge-
stellt. Standardchemotherapien sind oft
unwirksam, oder es kommt sehr schnell
zumRückfall. In einer Phase-II-Studiemit
dem oralen Quizartinib als Monotherapie
erreichten ++ von "" rezidivierten oder re-
fraktären Patienten mit FLT'-ITD-Muta-
tion eine Remission, die median !!,' Wo-
chen anhielt. Das mediane Gesamtüber-
leben betrug &',! Wochen. Ein Drittel der
Patienten konnte einer Stammzelltrans-
plantation zugeführt werden ["]. Quizar-
tinib ist ein oraler FLT'-Tyrosinkinasein-
hibitor (TKI), der sowohl bei mutierten
wie auch bei Wildtyp-FLT' aktiv ist.
Das Philadelphia-Chromosomkommt
bei CML und auch bei ALL vor. Es kodiert
für die BRC-ABL-Tyrosinkinase, die we-
sentlich für die Erkrankung verantwort-
lich ist. Die entsprechenden TKI Imatinib,
Dasatinib und Nilotinib haben die CML-
Therapie revolutioniert. Bei einigen die-
ser Patienten liegt allerdings eine T'!*I-
BCR-ABL-Mutation vor, die sie resistent
gegen die zur Verfügung stehenden TKI
macht. In einer Phase-II-Studie führte der
neue TKI Ponatinib bei CML-Patienten,
die entweder gegen die übrigen TKI resis-
tent/refraktär waren und/oder bei denen
bereits eine T'!*I-Mutation nachgewiesen
war, in **% zu einer „major cytogenetic
response“ und in *#% zu einer hämatolo-
gischen Response [!$].
Wenn die Standardprophylaxe der
Graft-versus-Host-Erkrankung (GVHD)
nach allogener Stammzelltransplantation
um den oralen Histon-Deacetylase-Inhi-
bitor Vorinostat ergänzt wird, lassen sich
Inzidenz und Schweregrad der GVHD
senken. Bei +* entsprechend behandelten
Patienten betrug die Inzidenz von GVHD
&&% im Vergleich zu +&% in historischen
Kontrollgruppen. Zu GVHD Grad III/IV
kam es in !'% vs. !"% in den Vergleichs-
gruppen [!!].
MLN"($# ist ein neu entwickelter ora-
ler Proteasominhibitor. Im Vergleich mit
Bortezomib hat die Substanz möglicher-
weise seltener neuropathische Neben-
wirkungen. Ergebnisse einer Phase-I/II-
Studie zur Kombinationstherapie von
MLN"($# mit Lenalidomid und Dexa-
methason in der Primärtherapie weisen
bei %* Patienten mit multiplem Myelom
eine ORR von "&% nach bis zu !& Kur-
sen dieser Kombination aus. Über die
Hälfte dieser Patienten erreichte mindes-
tens eine sehr gute partielle Remission,
&'% eine komplette Remission. Die Ra-
te und Qualität des Ansprechens verbes-
serten sich mit der Anzahl der Therapie-
kurse. Bei !#% der Patienten entwickelten
sich Hautausschläge (Grad '). Ansonsten
war die Verträglichkeit dieser rein oralen
Kombinationstherapie gut [!&].
